We ask that you fill out the Provider Availability Form (PAF) anytime there is an anticipated change in your schedule that might alter your current schedules with MOBHS or have additional availability.
We are always reviewing client and staff schedules so that we can identify cases that best suit the preferred area(s) you want to work in and the times you have available.
